We accept most major dental & insurance plans, but due to the complexity of some plans, please call our office for specifics.

Your dental insurance policy can be quite challenging to understand, so our goal is to assist you in maximizing your benefits.

Each employer pays an insurance premium for specific coverage, which fits the employers' budget. We encourage you to familiarize yourself with your plan's usual and customary (UCR) fee schedules, exclusions, deductibles, maximums, and required co-payments.

As a courtesy to you, our office will:

1. Research your dental insurance plan to advise you of benefits available to you.
2. Submit a claim to your insurance carrier within 24 hours of your visit.
3. Follow the American Dental Associations guidelines for coding procedures and submitting insurance claims.
4. Submit a Pre-Authorization to your insurance carrier before treatment is scheduled. However, it is your responsibility to request this service.
5. Provide you with an estimate for any recommended treatment.

Our expectations of you as the policyholder are:

1. To make your co-payment at the time services are rendered.
2. Understand that fee quotes by our office are strictly estimates based upon the information given to us by your insurance company.
3. Understand that your insurance policy is a contract between you, your employer, and the insurance company. We only provide insurance services as a courtesy.
4. Understand that insurance policies restrict payment for some procedures, use restricted fee schedules (UCR), and exclude some procedures based upon the amount of coverage purchased by your employer, and not by the necessity of treatment or our fees.
5. If your insurance carrier underpays or denies a claim you are responsible for the entire unpaid balance.
6. You are responsible for payment if your insurance company doesn't pay our office within 75 days.
7. Keep our office informed of any changes in your insurance coverage, employment, telephone numbers, and home address.
